Yes is for your to choose between weapon+joh or shield+joh.
If weapon is sword you will receive 2 swords

Fo can be substituted to ancients just then the pack amount will be +5euro (because of the diff between fo donation amount and ancient donation amount) so we can keep the same discounts as listed 15 euro discount for both simple and +JOH packs

No , sfo items since they do not have serials and are not covered by support they can not be in packs.
